I understand and agree that there are risks, foreseeable and unpredictable, associated with any exercise or education program. I am aware of these risks and agree that my participation is at my own risk. I hereby agree that Reform Fitness, LLC. or those employed by Reform Fitness shall not assume or have any responsibility or liability for the expenses or medical treatment or for compensation for any injury I or any family member may suffer during or resulting from participation in this program. I hereby discharge any and all rights and claims for damages that myself or other participants in my party may have or that may hereafter occur arising out of or in any way connected with my participation in this or any future Reform Fitness, LLC. program. I understand this Participant Release Form has important legal consequences and limits my ability to recover money if myself or other included participants are injured as a result of participation in this program. I have been given the opportunity to discuss its terms and consequences with an attorney of my choosing if I wish to do so. I also represent and warrant that all participants in this agreement and myself have been advised to seek consultation from a doctor about safely participating in this program and whether there are precautions or limitations to participation. When buying an item, you agree that: (i) you are responsible for reading the full item listing before making a commitment to buy it: (ii) you enter into a legally binding contract to purchase an item when you commit to buy an item and you complete the check-out payment process. The prices we charge for using our services / for our products are listed on the website. We reserve the right to change our prices for products displayed at any time, and to correct pricing errors that may inadvertently occur. To the maximum extent permitted by applicable law, Reform Fitness, LLC. assumes no liability or responsibility for any (i) errors, mistakes, or inaccuracies of content; (ii) personal injury or property damage, of any nature whatsoever, resulting from your access to or use of our service; and (iii) any unauthorized access to or use of our secure servers and/or any and all personal information stored therein. I grant permission to Reform Fitness, LLC. and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. I hereby release Reform Fitness and its legal representatives for all claims and liability relating to said images or video. I waive my right to any compensation. The Service and all materials therein or transferred thereby, including, without limitation, software, images, text, graphics, logos, patents, trademarks, service marks, copyrights, photographs, audio, videos, music and all Intellectual Property Rights related thereto, are the exclusive property of Reform Fitness, LLC. Except as explicitly provided herein, nothing in these Terms shall be deemed to create a license in or under any such Intellectual Property Rights, and you agree not to sell, license, rent, modify, distribute, copy, reproduce, transmit, publicly display, publicly perform, publish, adapt, edit or create derivative works thereof.
