I understand and agree that there are risks, foreseeable and unpredictable, associated with any exercise or education program. I am aware of these risks and agree that my participation, and my child's participation is at my and their own risk. I hereby agree that Reform Fitness and the Instructor shall not assume or have any responsibility or liability for the expenses or medical treatment or for compensation for any injury I or my child may suffer during or resulting from participation in this program. I hereby discharge any and all rights and claims for damages that my child(ren) or I may have or that may hereafter occur to me arising out of or in any way connected with my participation in this or any future Reform Fitness, LLC. program. I understand that the Participant Release Form has important legal consequences and limits my ability to recover money if my child(ren) or I are injured because of participation in this program. I have been given the opportunity to discuss its terms and consequences with an attorney of my choosing if I wish to do so. I also represent and warrant that my child(ren) and I have been advised to see consultation from a doctor about whether my child(ren) or I can safely participate in this program and whether there are precautions or limitations to participation. There are no refunds for Water Babies Classes or partial credits, and there is no guarantee that missed classes can be made up at a later date.
I authorize and consent to the use of his/her/my visual image by Reform Fitness for appropriate purposes, including but not limited to: still photography, videotape, electronic and print publications and websites. I give this consent with no claim for payment. No oral statements or promises a part of this agreement, which shall be governed by the laws of the State of Ohio.
